Realists 2026 Portfolio – RealSpine
THE POWER OF REALISM

A cadaver gives you one case, once. Train the same spine procedure until it is right.

RealSpine is a cadaver independent spine training platform, re-architected for 2026 and now in the United States. Repeat the exact pathology on demand, across the whole spine, with hyper realistic anatomy that bleeds, resists, and behaves like the real thing.

Designed and produced in Germany. Trained on in more than 50 countries. Partners include Stryker, Medtronic, DePuy Synthes, and Globus.

Surgical technology moved on. Training did not.

Spine surgery is one of the most common specialties in the world, and the way teams train for it has barely changed in a century. That gap is what RealSpine was built to close.

    01
    A cadaver cannot be repeated

You get one case, one time. You cannot standardize it, run the same scenario again, or summon a specific complication on demand. Fair assessment is almost impossible.

    02
    The OR raced ahead

Robotics, navigation, and minimally invasive technique advanced fast. The tools surgeons must master changed. The training infrastructure did not keep pace.

    03
    Programs cannot scale on cadavers

Supply, cost, and scheduling make readiness hard to standardize across cohorts and regions. Training ends up uneven, and so does field performance.

One platform that repeats until the technique is clean

RealSpine is a training environment, not a product demo. Physical models, radiation free visualization, and data driven feedback, so learning is measurable and transferable.

    Repeatable pathology on demand

Run the same case again and again, and across a cohort, so progression can finally be compared like for like.

    One platform, the whole spine

The Realists Fluid Pumping System powers bleeding capable training across lumbar and cervical, on a single pump built to last across product generations.

Buy once, grow as you go

Acquire the platform once, then add cartridges, modules, and pathologies as your program grows. Simpler to quote, easier to expand.

    Hyper realistic anatomy

Variable bone density, ligament tension, and tissue resistance, with bleeding and CSF leak realism. The feel that transfers to the OR.

    Cadaver independent

Train on demand without the supply, storage, cost, and scheduling burden of cadaver labs.

    Validated and German built

Designed, engineered, and produced in Germany. Internationally validated by surgeons before it goes to market.

    The Realists Fluid Pumping System

One pump powers every bleeding capable model in the catalog. It replaces a split hardware setup with a single platform, designed to last across product generations.

    • 1
Connect the platform. Mount the model, set parameters from one console.
    • 2
Select the cartridge. Anatomy specific cartridges drive flow and pressure for the procedure.
    • 3
Train at OR fidelity. Bleeding starts on cue. Reset between attempts.

Pressure, color, response time. The simulator behaves like the real thing.

    One platform. Three product families.

Out goes complexity. In comes a simplified portfolio you buy once and build on as your program grows. Lumbar, cervical, and deformity are covered from one platform, and every model plugs into the wider Realists Ecosystem.

    Fluid driven

    Five anatomies, one pump

Lumbar posterior, lumbar anterior-lateral, lumbar posterolateral, cervical anterior, and cervical posterior. Bleeding on request, all on the Fluid Pumping System.

    Modular

    Full construct, made modular

Three modular simulators scaled to the curriculum. From full modular access cervical to sacral, to a thoracolumbar and SI configuration, to a lumbar and SI variant for high volume residency programs.

    Deformity

    Deformity, now a line

Lenke 1 and Lenke 3 curve patterns on a shared base, plus patient specific models configured from real pre-op imaging.

    One ecosystem, every training context

The same platform adapts to how training really happens, from commercial teams to the OR pathway.

    For MedTech

Showcase, train, and validate technology in a realistic setting, so every demonstration earns adoption.

    For Hospitals and Associations

Deliver structured, repeatable training programs that build skill in less time.

    For Surgeons

Practice technique in realistic conditions, at your own pace, no operating-room required.

    For Researchers

Controlled, measurable simulation for research, validation, and objective performance analysis.

RealSpine is one part of a complete training system

    RealSpine

Hyper realistic physical spine simulation across the whole anatomy.

    RealImaging

Radiation free visualization that sharpens spatial orientation and planning.

    RealSmart

Data driven feedback that turns each session into measurable progress.

    RealLab

Mobile training environments that bring the ecosystem to courses and events.

Backed by a decade of work and the surgeons who train on it

RealSpine is a validated international platform now arriving in the United States

    50+
    Countries already training on RealSpine

Realists has spent a decade building a hyper realistic spine training ecosystem used by programs and OEMs worldwide.

„RealSpine is an excellent training model. It feels like operating a real patient with its authentic anatomical structures, the pathologies and the intra-operative bleeding system.“

    Dr. Roger Härtl
Professor of Neurological Surgery, Weill Cornell Medicine, USA

„RealSpine is an innovative spine surgery training model with realistic anatomy and flexibility, working well for MIS trainings on multiple pathologies.“

    Prof. Øystein P. Nygaard
Neurosurgeon, St. Olavs University Hospital, Norway
